Up One Level Case Form

Cases are like mini-folders containing like activity in a person's file. A physician might track all activity relating to a patient's back problem in one case, and to the patient's "Asthma" activity in another

Another example would be a group of cases that arise out of a multidisciplinary treatment plan. The Case Manager might develop a treatment plan that involves counseling, medication, vocational rehabilitation and family therapy activities. Information relating to each of these activities would be kept in separate case folders as part of the individual treatment plan (case) folder. The Case Manager can monitor and control all of these activities limiting the hours or dollars spent on some while extending the limits on others. The Case Manager might also set hour or dollar limits for a group of individuals (see Group Object). This might be necessary when an insurance company or other funding agency limits the time or dollars that can be expended for a particular activity by members of the group (case limit) or limits such expenditures for individual members over all activities (individual limit)

By definition - a case is a folder used to define, store and organize related activity.
